Patient Resources

For Patients

We have prepared this information to assist you in your visit to our office. As always we will assist you in anyway possible with questions or concerns. Please let us know how we may better serve you. At the Idaho Ear Clinic, we strive to see our patients in a timely fashion, but occasionally circumstances arise that may delay your appointment. We apologize in advance if your appointment is delayed and hope you understand that we do value your time.

What to Bring to an Appointment

  • Keep all of your scheduled appointments. You must cancel / reschedule an appointment 24 hours prior to the appointment.
  • Bring all paperwork to your appointment filled out and ready to give to our receptionist. You may complete the paperwork on our website to print and bring with you.
  • Notify our front desk staff of any changes in your address, contact phone numbers, insurance or name changes prior to seeing the physicians. When these are incorrect it is more likely your bill will not be paid by your insurance company.
  • You must bring a photo id and your insurance card to every visit.
  • Co-payments and balances are due at time of service. There is a $15 charge for not paying your co-pay. You have agreed to this co-pay with your insurance company and we cannot bill this.

Billing and Insurance

FINANCIAL POLICY

We work to provide the best, and most appropriate, treatment possible for our patients. We understand the need for a clear understanding between the patient and his/her doctor regarding financial arrangements for medical and surgical care.
The doctors of the Idaho Ear Clinic have established fees for professional services to their patients. The responsibility for payment of those fees is the direct obligation of the patient. Full payment for all co-pays, co-insurance, or any deductible amounts is due at the time of service. If you require financial assistance from our office or want to arrange a payment plan, please contact our billing office at 208-938-5823. We accept cash, check, Visa, MasterCard, American Express, Discover, and Care Credit.

We also ask that if you must cancel and appointment or surgery, please do so at least 24 hours prior to your appointment time. If you fail to show with canceling, or if you arrive more that 20 minutes late for your appointment, you will be charged a no-show fee of $50. Idaho Ear Clinic uses third party agencies to collect on accounts that are past due. Returned checks are subject to an additional fee and will be sent to a third party agency if not paid in full after receiving notification.
